Notarization of Documents
NOTARIZATION OF DOCUMENTS
(As of 01 January 2025)
Submission of documents/requests is strictly from 8:30 AM until 2:00 PM only.
Processing of documents takes three (3) working days upon receipt of complete requirements.
Releasing of documents is strictly from 3:00 PM to 4:00 PM only.
The following documents can be notarized by the Embassy:
- Affidavit (General)
- Affidavit of Loss of Passport
- Affidavit of Mutilation/Destruction of Passport
- Affidavit of Support and Invitation
- Affidavit of Support and Consent with Special Power of Attorney
- Certificate of Nationality
- Name Certification
- Passport Issuance Certification
- Special Power of Attorney
The following are the requirements for the notarization of documents:
- Personal appearance of the applicant/signatory and witnesses.
- Duly-accomplished affidavit/SPA form

** Physical forms are available at the the Consular Section of the Embassy
- Original and photocopy of applicant/signatory and witnesses’ valid passports.
* Additional documents for Affidavit of Support and Invitation:
- Copy of TRC / Work Permit / Labor Contract
* Additional documents for Name Certification:
- Original and copies of Philippine documents (which reflects different arrangements of names)
*Additional documents for Certificate of Nationality for Newborns:
- Notice of birth from the hospital (notarized with English translation)
- Copies of parents’ passports
- Photocopy of the passports of the parties named in the Affidavit or the authorized Attorney-in-Fact
NOTE: Consular officers may require additional documents on a case-by-case basis.

Report of Death
Requirements for Report of Death
As of 22 April 2025
*Note: Reports of Deaths can only be processed at the Philippine Embassy in Hanoi.
Present original documents and submit four (4) photocopies of each (collated and
arranged into 4 sets according to the order of documents below):
Basic Requirements;
1. Report of Death Form (https://hanoipe.dfa.gov.ph/images/2018/Report-of-Death-ROD-Form.pdf)
● Four (4) forms should be printed in A4 paper, originally filled out
● Entries must be TYPEWRITTEN
● Entries must be at the time of death of the deceased/subject
2. Consular Mortuary Certificate
● Issued by the Philippine Embassy in Hanoi or Philippine Consulate General in
Ho Chi Minh City
3. Death Certificate issued by the country where the death occurred (original copy
should be submitted to the Embassy)
a. Issued by the Vietnamese Civil Registry Office (People’s Committee) with
notarized English translation, and authenticated by the Ministry of
Foreign Affairs of Vietnam (one original with 3 photocopies).
4. Record of Corpse Embalming with notarized English translation
5. Health Quarantine Certificate with notarized English translation
6. Photocopy of the Philippine passport of the deceased
7. Photocopy of the passport of the informant
*Note: Upon evaluation, additional documents may be required by the evaluating desk
officer from the applicant, including documents related to the shipment of the remains
of the deceased.
Fees
1. Consular Mortuary Certificate: 650,000 VND
2. Report of Death Application Fee: 650,000 VND
Turnaround Time: 10 working days
Travel Documents Requirements
As of 8 April 2025
The Travel Document (TD) is issued for emergency purposes to Filipinos who are in Vietnam due to the following reasons:
- Filipinos who lost their Philippine passports while travelling in Vietnam;
- Filipinos with expired passports who must return to the Philippines immediately; and,
- New born Filipino children who must travel to the Philippines immediately.
You may secure a TD from the Philippine Embassy in Hanoi or the Philippine Consulate General in Ho Chi Minh City. The TD is valid for one-way travel back to the Philippines only.
Requirements:
- Personal appearance of applicant;
- Duly accomplished Travel Document application form (available at the Embassy/Consulate);
- Photocopy of the data page of the lost Philippine passport;
- Four (4) recent passport size ID photos (3.5cm x 4.5cm) colored with white background;
- Notarized police report with English translation; and
- Affidavit of loss (available at the Embassy/Consulate).
- For new born Filipino children, a Report of Birth (ROB) is required before the issuance of TD. For the requirements and procedures of ROB, kindly visit the link below:

Consular fees:
Travel Document: 780.000 VND
Affidavit of Loss: 650.000 VND
